Dr Chong completed her medical degree at Monash University and completed her specialty training through Monash Health, Eastern Health and the Alfred Hospital. Dr Chong has a Fellowship in continence management.
Dr Chong works in the hospital as the Medical Lead in GEM at home, a service that provides geriatric assessment and rehabilitation for older patients at home. She also provides care for older people in complex care clinic and with aged care assessment service (ACAS). She is also a current member of the council for Australian and New Zealand Society for Geriatric Medicine (ANZSGM).
She is passionate in improving the health and quality of life for her patients, advocating for her patients including helping them to stay at home. Her strengths are providing complex care for patients in the community including managing polypharmacy, cognitive impairment, falls and balance, chronic wound care. She is also the co-author of the Geriatric Society's position statement on Immunisation for older people.
Dr Chong is fluent in multiple languages including Mandarin, Cantonese, Hokkien and Malay.

Dr Lac is a senior geriatrician. She was an adjunct senior lecturer for Monash University (2014-2016) and has been heavily involved in the supervision and training of geriatrics trainees in her multiple public hospital positions around Victoria.
She has extensive experience and interests in all geriatrics conditions, including:

Dr Gupta graduated from the University of Melbourne and completed his training across Eastern Health and Monash Health. He holds a substantial public hospital appointment as a Geriatrician, Perioperative and General Medicine Physician at Monash Health.
Dr Gupta has expertise in all core areas of Geriatric Medicine, with special interests in cognitive impairment and dementia, residential aged care, falls prevention, chronic disease management, palliative care and perioperative medicine in the elderly. During his training he has also gained experience in several relevant areas including chronic pain, movement disorders, metabolic bone disorders and acute medicine.
He provides holistic, compassionate, and collaborative care that is catered to the unique needs of the older adult and aimed at improving quality of life.

Dr Co graduated from Monash University with a Bachelor of Medicine and Surgery in 2014. Her further training includes qualifying as a Fellow of the Royal Australasian College of Physicians as a Consultant Geriatrician in 2022, with a Graduate Certificate in Health Promotion (Curtin University) and a Clinical Diploma in Palliative Care (Royal Australasian College of Physicians).
Dr Co has varied experience in community-based and hospital medicine, in metropolitan Melbourne as well as in country Victoria. She trained at Bairnsdale Regional Health Service, Monash Health, and Alfred Health. Dr Co has experience working across many areas of Geriatric medicine, including Geriatric Evaluation and Management unit (GEM), GEM at Home, Residential In Reach, Aged Psychiatry, Palliative Care, Aged Care Assessment Service (ACAS), Geriatric Consults; Falls, and Memory clinics. She is eager to assist patients achieve better health and quality of life holistically and as guided by the patient’s personal priorities in different stages of life.
Dr Co’s special interests include: Chronic disease management, Provision of supportive care and Optimising quality of life, De-prescribing and Medication rationalisation, and coordinating with patients, their GP, and other specialist/s, as well as family supports to achieve the best health outcomes.
She is fluent in English and Tagalog (Filipino).
